What is midface filler?

Midface filler, also called cheek filler, uses cross-linked hyaluronic acid gel to restore selected areas of volume and contour around the cheeks.

The assessment considers cheek projection, the lower eyelid transition and the nasolabial folds together.

The aim is a coherent facial shape, with an amount chosen for your anatomy.

Where is the cheek area?

The Thai heading asks which part of the face is called the cheek or “apple of the cheek”. The image points attention to the contour below the eye and beside the nose, which the doctor considers alongside expression and surrounding support.

Where is the apple of the cheek?

The apple of the cheek is the front-cheek area that becomes more prominent when you smile.

An oval contour follows the shape of the individual face.

Cheek size, under-eye support and the folds beside the nose must be considered together, including how they look in movement.

When is an assessment useful?

You may want advice about naturally flat cheeks, a loss of cheek support with age, hollows below the eyes or a marked cheek-to-fold transition.

Both women and men can discuss these concerns.

Filler cannot correct every cause of skin laxity, and the doctor may recommend a different approach.

Preparing for filler treatment

The Thai preparation graphic covers medical and allergy history, medicines and supplements that may affect bruising, and avoiding alcohol before the visit. Bring a complete medicine list. Do not stop prescribed medicines on your own: any change should be agreed with the prescribing clinician and treating doctor.

First 48 hours

Let the area settle

Swelling and bruising can occur. Use a gently applied, cloth-wrapped cool compress as advised by your clinician. Do not apply ice directly or press hard. Contact the clinic if swelling, redness or pain is increasing.

1–2 weeks

Protect the treated area

Avoid impact and strong pressure on the area. Follow the doctor’s advice about facial movement and exercise.

At least 2 weeks

Pause facial treatments

The clinic advises pausing facial massage, scrubs and facial treatments for at least two weeks. Check with the treating doctor before scheduling another procedure.

2–4 weeks

Check heat-based treatments

Discuss the timing of facial lasers, steam rooms, saunas and hot baths before treatment. A 2–4 week interval may be advised for some activities; your clinician should specify which restrictions apply to your product and planned procedures.

Know what to watch for

The clinic’s guide advises avoiding alcohol for two weeks, or at least 48 hours. It also lists raw and fermented foods as clinic-specific restrictions. Follow your personalised instructions. Severe unusual pain, vision changes or pale/dusky skin require immediate medical attention.

Where is the midface, and how much fullness is appropriate?

The midface includes the cheek region between the lower eye and upper mouth area. “Apple cheek” describes part of this contour. Assessment considers its relationship to the under-eye transition, nasolabial fold and cheekbone rather than simply making the cheek rounder.

Product and amount depend on tissue support and previous treatment. More volume is not always more natural, and a photograph of someone else cannot determine your plan.

How should I prepare for midface filler?

Bring previous treatment dates, products and photographs if available. Tell the doctor about medicines, supplements, allergies, medical conditions, pregnancy or breastfeeding, and any infection or irritation near the area. Ask about the intended product, alternatives, cost and follow-up before consenting.

Some medicines and supplements can affect bruising or bleeding. Do not stop prescribed medicines yourself; any change should be agreed with the prescribing clinician. Ask the clinic about alcohol, exercise and other preparations relevant to your appointment.

What should I do after filler?

Follow your treating doctor’s instructions for skin care, activity, pressure on the area and the timing of other procedures. Mild swelling, tenderness or bruising can happen. Avoid firm massage unless the clinician specifically advises it, and ask how and when to use a cool compress.

Contact the clinic for increasing swelling, persistent pain, redness, warmth or a new lump. Severe unusual pain, pale or dusky skin, or vision changes need immediate medical attention. Keep the urgent contact details and attend the agreed follow-up; an early swollen photograph is not the final result.

Will cheek filler make my face rounder?

That depends on your starting point and the plan. Tell your doctor if you want to avoid extra fullness and ask how the proposed change relates to your whole face.

No. They are assessed together, but that does not mean both require treatment. A plan may focus on one area or be staged.

A broad 6–24 month range may be discussed, depending on the product and individual circumstances. This is an estimate, not a reason to add filler at a fixed interval. A review should check your contour and any remaining filler.

In selected faces, improving cheek support can soften the transition toward the nasolabial folds. It does not mean every fold needs direct injection or that every patient needs cheek filler. The doctor should assess the cause of the fold first.

The plan follows your assessment. Ask for the product, proposed amount, total cost and any follow-up charges before agreeing to treatment.

Bruising, swelling and tenderness can occur. Ask your doctor about recovery for your particular plan and leave flexibility around important events.
