Under-eye filler is an injectable hyaluronic acid (HA) gel used to soften selected hollows beneath the eyes.
It can improve the transition between the lower eyelid and cheek when volume loss is part of the concern.
At MICHIKO in Bangkok, the starting point is understanding whether you are seeing a hollow, pigmentation, puffiness or a combination.
Assessment considers five contributing factors: inherited anatomy and pigmentation, allergies and rubbing, irritation when removing makeup, age-related changes in bone and fat support, and visible vessels associated with inadequate rest.
These causes need different care.
More filler is not the answer to every dark circle.
The illustration places the under-eye concern in the context of skin and supporting layers. Hollowness, pigment, puffiness and muscle prominence do not have the same cause, so adding volume is not the answer to every dark-circle concern.

Pigmentation, irritation and thin skin can also create darkness.
Filler may soften a contour-related shadow, but it does not remove pigment or treat an allergy.
Your plan may instead include skin care, managing irritation or another treatment.

Tell the doctor about aspirin, anti-inflammatory medicines, blood thinners, fish oil, vitamin E and herbal supplements. The clinic’s guide asks patients to review these ahead of treatment. Do not stop prescribed medicine without your prescriber’s advice.
Bring details of regular medicines, allergies, illness, previous injections and implants. Tell the team if you are pregnant or breastfeeding, have an active infection or a history of cold sores.
The clinic advises avoiding alcohol for 24 hours before treatment. Arrive with a clean face and allow time to discuss your questions, product choice, cost and follow-up.
Swelling and bruising can occur. Use a gently applied, cloth-wrapped cool compress as advised by your clinician. Do not apply ice directly or press hard. Contact the clinic if swelling, redness or pain is increasing.
Avoid impact and strong pressure on the area. Follow the doctor’s advice about facial movement and exercise.
The clinic advises pausing facial massage, scrubs and facial treatments for at least two weeks. Check with the treating doctor before scheduling another procedure.
Discuss the timing of facial lasers, steam rooms, saunas and hot baths before treatment. A 2–4 week interval may be advised for some activities; your clinician should specify which restrictions apply to your product and planned procedures.
The clinic’s guide advises avoiding alcohol for two weeks, or at least 48 hours. It also lists raw and fermented foods as clinic-specific restrictions. Follow your personalised instructions. Severe unusual pain, vision changes or pale/dusky skin require immediate medical attention.

Under-eye darkness can involve pigmentation, thin skin, visible vessels or a shadow from a hollow. A bag or tendency to fluid retention adds another consideration. Filler may address selected hollows but does not treat every cause of darkness or remove a bag.
Tell the doctor about morning puffiness, previous filler and any surgery. Product selection, tissue thickness and midface support matter; adding volume to an already swollen area may worsen its appearance.
The right role for each approach.

A hollow, a protruding fat pad and dark pigmentation can each create a tired appearance. HA filler may suit a selected hollow but does not remove excess skin or a prominent fat bag. Fat transfer uses your own tissue and involves harvesting, with different recovery and variability. Surgery may be more relevant for a significant fat bag or excess skin. None is universally safer for every patient.
A roll that appears beneath the eye when smiling can involve the eyelid muscle rather than a missing volume. Filling it without assessing movement can increase puffiness. Discuss the cause before deciding on the method.
Know what comes before and after.

Bring previous treatment dates, products and photographs if available. Tell the doctor about medicines, supplements, allergies, medical conditions, pregnancy or breastfeeding, and any infection or irritation near the area. Ask about the intended product, alternatives, cost and follow-up before consenting.
Some medicines and supplements can affect bruising or bleeding. Do not stop prescribed medicines yourself; any change should be agreed with the prescribing clinician. Ask the clinic about alcohol, exercise and other preparations relevant to your appointment.
Follow your treating doctor’s instructions for skin care, activity, pressure on the area and the timing of other procedures. Mild swelling, tenderness or bruising can happen. Avoid firm massage unless the clinician specifically advises it, and ask how and when to use a cool compress.
Contact the clinic for increasing swelling, persistent pain, redness, warmth or a new lump. Severe unusual pain, pale or dusky skin, or vision changes need immediate medical attention. Keep the urgent contact details and attend the agreed follow-up; an early swollen photograph is not the final result.
Thin under-eye skin makes gel properties and placement particularly important. The proposed HA model should be discussed in terms of tissue integration and swelling tendency, rather than brand alone. Longevity varies with product, anatomy and previous treatment; a persistent bulge is not evidence that another syringe is needed.
Early swelling can settle, but persistent, recurrent or asymmetric puffiness needs examination. The doctor may assess the cheek transition, existing filler or other causes before discussing observation, dissolving HA or another approach. Severe pain, skin colour change or visual symptoms require urgent medical care.
